Job Details The Home Health Aide Coordinator responds appropriately to the special needs of every patient while maintaining confidentiality and compliance with the HIPAA Compliance Program. Responsibilities include, but are not limited to: Coordinating the placement of home health aides as indicated by the hospice nurse Working with home health aide vendors to place aides as needed Maintaining updated vendor aide files; NYS HHA Certificates, Chauncey Reports, in-services, etc. Interacting with patients and family members regarding aide placements, scheduling, and addressing any problems or issues The ideal candidate will have prior experience as a home health aide and will possess a basic understanding and commitment to the hospice concept of care for the terminally ill. Minimum Requirements: High school diploma or equivalent required Commitment and empathy to patients and family Good communication and pc skills Salary Range USD $25.00 - USD $29.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
