POSITION SUMMARY

Under the supervision of the Director of Nursing, the LNA / Phlebotomist position is responsible for phlebotomy specimen collections in all areas of Woodridge. He / She will serve as a mentor to all other phlebotomy staff by assisting in training and mentoring, according to CVMC policies and procedures. When phlebotomy services are completed, he / she will function as the float LNA, providing direct and indirect nursing care for residents on any of the units within Woodridge.

DUTIES:

A. Identifies and prioritizes residents who require phlebotomy services. Collaborate with the Nursing Team to perform phlebotomy services within timeframe indicated by providers.

B. Perform venipunctures on all adult residents. Uses standard technique to obtain the adequate volume of blood necessary for testing. Document collection in the appropriate electronic medical record and labels specimen according to CVMC procedure.

C. Collects urine specimens on all adult residents. Provide resident with appropriate container and collection instructions specific for each test. May need to assist the resident with collection. This is dependent on the cognitive impairment of the resident.

D. Follows-Up on any problems or any pending specimens. If possible, resolve any pending issues prior to the end of shift, ensuring that communication occurs with the clinical team overseeing the resident's care.

E. Completes daily care needs of residents, according to residents’ plan of care and within the scope of individual’s ability and other guidelines listed in position summary. Daily care needs may include application of slings, routine feedings, range of motion exercises, assistance with ambulation, complete and partial baths, total personal hygiene care, position changes, bed changes, skin care and assisting physicians with examinations.  

F. Prepares residents for meals, distributes food trays, assists or feeds residents as necessary, removes trays and documents intake.  Anticipates residents' needs whenever possible and responds to call lights. 

G. Takes and records temperature, pulse, respiration, weight, height, intake and output, and blood pressure, as requested. 

H. Observes any overt changes in condition or behavior of residents and reports such to a licensed nurse.  Documents in the medical record any resident care activities. 

I. Handles and cares for residents' clothing and other personal property such as prosthetic devices, hearing aids, dentures, glasses, contact lenses, etc… 

J. Helps prepare resident room and immediate environment for admissions.  Assists in and, as necessary, accompanies resident through admission, transfer, or discharge procedure. Prepares residents for appointments and accompanies them to appointments as needed.

K. Encourages residents through positive motivation to achieve the highest level of functioning as is practicable for resident.  Strives to provide care in a manner that leads to a high level of satisfaction. 

L. Contributes to the nursing process through observation, reporting and recording of changes in residents’ level of functioning including physical/cognitive/emotional status.

M. Collaborates with co-workers and other departments to constantly seek ways to improve quality of care and increase resident and family satisfaction.  

N. Assists in providing a safe and secure environment for the resident, personnel any visitors by compliance with Woodridge policies, such as Infection Control, Fire and Electrical Safety. Reports out of order equipment and other problems to licensed nurse.

O. Serves as a positive role model and preceptor for new employees.   

P. Employees will perform his/her job in a safe manner as defined in any and all applicable CVMC policies specific to the job including but not limited to policies addressing: Universal precautions, Use of PPE, Safe lifting/ergonomics, Handling of hazardous materials. 

Q. Notifies direct supervisor of infractions of policy, procedure, laws and regulations as they are identified.  Notifies supervisor’s manager of same issue if supervisor does not correct or resolve issue within a reasonable time frame.  Notifies Corporate Compliance Officer of any issues that continue to be unresolved.

R. Performs all other duties as assigned.

COMMUNICATION:

S. Promotes positive communication among staff from all departments.       

T. Participates in unit staff meetings 75% of the time to enhance communication and improvement of resident care.  

EDUCATION:

U. Assess personal learning needs and shares this with the Nurse Manager when appropriate for the development of a program to meet identified needs.

V. Attends In services and Continuing Education Programs.       

MINIMUM REQUIREMENTS

Successful completion of approved phlebotomy course and venipuncture experience a must. Certification in Phlebotomy strongly preferred. Registered in the State of Vermont as a Licensed Nursing Assistant. Must be at least 18 years of age.

 EXPERIENCE

Prior experience as a Phlebotomist and Licensed Nursing Assistant in a health care setting is preferred.

Must demonstrate ability to perform basic nursing care skills.

INDEPENDENT ACTION

Generally establishes own work plan and priorities in conformance with established policies and standards.  Problems lacking clear precedent are reviewed with a licensed nurse.
