Must-Have:

- Minimum of one year working with the frail or elderly population strongly preferred
- Valid driver's license, proof of insurance, and means of transportation

Job Description:
- Provide primary medical coverage in collaboration with physicians as needed
- Perform initial, semi-annual, and annual assessments, and additional assessments of participants as needed
- Implement appropriate care plans for participants
- Participate in discharge planning for acute and long-term placement
- Evaluate and treat participants during acute illnesses
- Prescribe medications, therapies, and other treatments prudently
- Consult with the Primary Care Physician for specialist referrals as indicated
- Conduct nursing home or home visits when needed
- Collaborate with the Medical Director and Clinic Supervisor to formulate clinical policies, procedures, and standards of care
- Assist with the development of standards of care, ongoing monitoring, and evaluation of patient care practice and service delivery
- Provide guidance and training to staff regarding medical and quality assurance issues
- Advise the Medical Director and/or Primary Care Provider on enhancing accountability of services to participants and referral sources
- Act as a resource during intake of new participants and contribute to the day-to-day operation of the center programs
