RN MDS Case Manager at Providence Mount St. Vincent

The TC Case Management Nurse fulfills the Mission of Providence Health & Services in working with patients, families, volunteers, and PMSV staff. PH&S Core Values of respect, excellence, compassion, justice, and stewardship are demonstrated by the Case Management Nurse in her/his daily work. Nursing care is holistic and is delivered in accordance with the PH&S vision, the PMSV Statement on Professional Nursing Practice, relevant professional nursing standards (including the Pioneer Network's Nurse Competencies in Culture Change), and the Washington State Registered Nurse scope of practice.

Required Qualifications:

  • Upon hire: Washington Registered Nurse License
  • Two years: Nursing experience in LTC, TCU or equivalent experience

Preferred Qualifications:

  • Bachelor's Degree: Nursing

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
