Description
Provides direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor. These services are provided in the states of Oregon and/or southwest Washington.

Required Qualifications:
- Education to meet certification, license or registration requirement
- Oregon Registered Nurse License upon hire or
- Washington Registered Nurse License upon hire or
- National Registered Nurse Compact License upon hire
- National Provider BLS - American Heart Association upon hire
- Unencumbered Registered Nurse License in state(s) of hire, and wherever care is delivered upon hire. If RN holds a multi-state license through the Nurse Licensure Compact (NLC), the RN must adhere to the requirements of licensure in the new state(s) of hire
Preferred Qualifications:
- 1 year experience with acute hospital care, infusion, or home health

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
