The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.
• Leadership: Uses problem solving skills to identify and work through issues. Facilitates quality of patient care. Serves as an advocate for the patient. Serves as liaison for various clinical functions such as Clinical Practice, Occupational Health, etc. • Assessment: Demonstrates refined assessment skills to collaboratively discover the patient's story. Integrating the following elements: subjective data, objective data, physical picture, psychosocial components, personal preferences and norms, and learning needs and preferences. Must be able to utilize an ongoing priocess to interpret data and recognize patterns and trends to determine both immediate and future care needs. • Planning: Dialogues with the patient/chosen support system to identify what is important to them and incorporate his/her preferences and norms into the individualized care plan. Engages the interdisciplinary care team in planning individualized care. Identifies relevant resources based on necessity and availability. Determines immediate needs and establishes realistic short term and long term goals based on desired outcomes. Prioritizes interventions to advance the patient toward optimal health or support a peaceful death. Constantly considers ongoing modifications of care plan based on patient's needs, capabilities, and condition. • Implementation of interdisciplinary Plan of Care: Provides age-specific patient care by assessing, planning, implementing, evaluating and documenting the individual patient needs, actions taken and the patient responses relative to nursing care. Accepts direction from provider. Reviews orders for clarity and appropriateness. Acts with patient advocacy at the core of nursing care when implementing the interdisciplinary plan of care. Collaborates with healthcare team to educate patient and their support system. Identifies patient learning needs, incorporates into care by providing education and counsel to patients, and documents patient understanding. Analyzes, modifies and implements strategies to progress the patient through their plan of care. Ensures completion of nursing interventions, medical orders, and other related therapies, using approved policies/procedures/protocols. Possesses the ability to prioritize job duties according to patients' needs and changing accuity levels. Performs patient transfers and transports patients via wheelchair. Ability to lift and carry various equipment. Ability to provide routine nursing care such as start IV's, perform dressing changes, etc. for multiple patients in a timely manner. Ability to perform in critical situations including administering CPR. Helps to facilitate care provided is consistent with policies, procedures and telephone triage protocols, in cooperation with the physician. Able to function in emergency situations and facilitates the care provided for all patients. Makes patient referrals as directed by the provider, and as appropriate, to resolve identified problems or reach established patient goals. The role involves working in a team based patient centered environment. • Evaluation: Continuously assesses patient's progress toward goals and outcomes. Seeks input from patient and their chosen support system regarding progress toward goals. Analyzes and modifies plan of care with patient and interdisciplinary team as indicated to achieve desired outcomes. • Communication and Documentation: Communicates individualized plan of care and patient's progress toward goals to all members of the care team including the patient and his/her chosen support system. Consistently utilizes apppropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care. Documents appropriate information in a timely manner. • Delegation: Appropriately delegates to ensure the implementation of the plan of care while ensuring patient safety, considering the nature of the task, competence of the delegate, and the patient's response. Demonstrates commitment to quality patient care by holding self and others accountable for optimal role performance. Delegates or assigns patient care to appropriate health care team members commensurate with their validated competency to perform the task, evaluates outcomes. Teaches clinical team, and provides feedback, as needed. Assists in orientation of new staff and services as a resource person and professional role model, within scope of practice. Assists Clinic Administrator, Clerical Supervisor, and Lead Clinician with assuring competencies of clinical staff. • Performance Accountability/Evidence-Based Practice: Demonstrates independence in the foundation of nursing care practice within this specialty. Practice according to your state's Nurse Practice Act, ANA Code of Ethics and ANA Bill of Rights. Seeks involvement in Shared Governance through council participation and/or by staying current with council activities. Identifies opportunities for process improvement, incorporates evidence-based practice and research. Engage in self-reflection and develop personal goals to enhance knowledge, obtain professional certification or pursue academic degree as strategies to promote professional growth. Integrate gained knowledge into practice. Maintain professional portfolio. • Teamwork: Develops and maintains positive relationships with team members/internal customers. Consistently demonstrates a willingness to adjust activities to accommodate the needs of team members/internal customers. Develops team members through mentoring, precepting, orientation, and providing feedback. Resolves conflicts independently, as appropriate. Accepts responsibility for actions and responds positively upon feedback from others. Utilizes data received from patient satisfaction and practice performance results to identify opportunities to improve and participates in the planning and improvement process. • Clinical Lab Duties (where applicable): Specimen collection and testing analysis: Performs venipuncture, skin puncture and lab procedures with attention to accuracy by checking patent ID and DOB. The employee understands and follows HIPAA guidelines and ensures the confidentiality of patient results. Reporting results: Ensures all test results, in-house and referred, are reported to the provider by checking a manifest or providing oversight of pending labs. The employee documents or assists in documentation or corrective action needed for Quality assessment, Proficiency Testing and Quality Control. Documentation: Ensures all routine, daily, monthly, and annual maintenance and QC are performed, documented and kept for the appropriate number of years • Women Services Specific: Maintain education in electronic fetal monitoring for competency in interpreting fetal non-stress tests that are scheduled on the nurse schedule. Ability to complete OBGYN triage phone calls based on evidence based protocol. • Human Experience: Show courage through creating and sharing innovative ideas to improve the experience for both patients and peers. Round on patients to create meaningful connections and keep patients informed of visit details (delays/wait times). Model the experience principles through consistently engaging in Always Event behaviors and viewing feedback through the patient lens. Recognize and value the unique differences and similarities in both our team members and patients to create an inclusive environment where diversity is celebrated. Explain all processes to patients in plain language and utilize teach back to ensure understanding. Qualifications
