The RN Case Manager will work as a care team member to support the needs of the clinics and assume responsibility for the ongoing case management of an assigned panel of patients. As a case manager, the RN will coordinate with a multidisciplinary team that includes patients, families, physicians, medical assistants, social workers, counselors, and other clinic support staff to provide high quality care.

Required Qualifications:
- Graduation from an accredited school of nursing.
- Alaska Registered Nurse License upon hire.
- 2 years of Professional nursing experience.

About the Team
Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
