ChristianaCare is currently seeking a full-time Licensed Practical Nurse for our Transitions of Care team to work hybrid from home and at our Avenue North location in Wilmington, DE.
This position will assist the RN and providers in the coordination of care to patients transitioning from acute care to home.
**To be successful in this role, you must be able to work well independently. Previous remote work experience is helpful.
WORK SCHEDULE:
Monday-Friday, 8:00am-4:30pm with some late day rotation.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Demonstrates knowledge & skills necessary to provide care coordination appropriate to pediatric, adolescent, adult, and geriatric patients.
- Works in conjunction with referral sources to identify patients appropriate for hospital follow-up.
- Identifies patient needs and escalates to next appropriate resource as warranted.
- Communicates patient needs/requests to appropriate caregivers.
- Reviews discharge instructions with patient.
- Reviews care plan with patient as well as patient’s care team.
- Schedules any necessary follow-up visits.
- Collaborates with other members of the care team.
- Provides scripted patient education.
- Receives delegated tasks from RN.
- Participates in process improvement strategies.
- Participates in a relationship-based care approach to care delivery.
- Facilitates and participates in daily team huddles every morning.
- Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
- Performs other related duties as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- High school graduate or equivalent Graduate of an accredited school of practical nursing.
- Three to five years physician office or clinical experience preferred.
- An equivalent combination of education and experience may be substituted.
SPECIAL REQUIREMENTS:
- LPN multistate license is required to cover DE, NJ, & MD
- Must also be licensed to practice as a LPN in PA
