This Metabolism Clinical Social Worker will be responsible for:
- Performing biopsychosocial and risk assessments, developing and implementing interventions and treatment plans
- Providing advocacy and complex resource coordination
- Providing psychosocial consultation to the medical team and planning and implementing special events and programs aimed at providing psychosocial education to patients/families and connection to social support.
To qualify, you must have:
- Master’s Degree in Social Work, and MA LCSW or LICSW is required.
- Experience in medical environment or with chronic illness is preferred
- Knowledge of child development, excellent communication skills, flexibility and ability to work in fast-paced environment.
- Bilingual/bicultural/diverse candidates are encouraged to apply
