Flexible territory with focused routes to support work-life balance Continuing education and professional growth opportunities Meaningful one-on-one patient care Occupational Therapist Responsibilities As a Home Health Occupational Therapist , you will: Complete in-home Occupational Therapy evaluations and reassessments Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization Recommend adaptive equipment, home modifications, and fall prevention strategies Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team Document patient progress accurately using EMR while maintaining Medicare and agency compliance Current Arizona Occupational Therapist (OT) license Graduate of an ACOTE-accredited Occupational Therapy program Minimum one year of Occupational Therapy experience (Home Health preferred) Valid driver’s license, reliable transportation, and current auto insurance
