Responsibilities:
Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage membersEvaluate patients prior/current medical histories, chronic conditions, and medicationsEducate patients about their health conditions and prescribed treatmentsIdentify care gaps, document findings, and provide recommendationsNo requirement for treatment plan creation or medication prescribingDesired

Qualifications:
