Work Environment

This role operates in a fully remote setting, allowing for flexibility in managing your schedule while maintaining a strong focus on client interactions. The typical work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required to meet specific client needs. A flexible approach is essential, as on-call availability may be needed to respond to urgent situations promptly.

Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs to identify their emotional, mental, and social requirements.
  • Collaborate with clients and their families to devise customized care plans that align with their individual goals and preferences.
  • Provide counseling and emotional support, facilitating discussions around life transitions such as end-of-life decisions or coping strategies for loss.
  • Advocate for clients' rights and ensure access to community resources that support their care and overall well-being.
  • Work closely with an interdisciplinary team that includes nurses, caregivers, and other healthcare professionals to ensure coordinated care and address any emerging concerns.
  • Perform regular check-ins with clients to monitor their health and living conditions, proactively addressing safety concerns or changes in their status.
  • Facilitate family meetings to enhance communication and manage conflicts related to care decisions.
  • Educate clients and their families about available resources, support groups, and programs aimed at enhancing social engagement and improving quality of life.
  • Maintain precise and timely documentation of assessments, care plans, progress notes, and relevant client communications.
  • Stay current on best practices in social work, ensuring compliance with ethical standards and relevant regulations.

Qualifications
