Crossroad Behavioral Health Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team in a remote capacity, working closely with school settings in Phoenix, Arizona. Key Responsibilities

As an LCSW, you will play a critical role in enhancing the well-being of students by:

  • Conducting thorough assessments of counseling needs for students, diving deep into individual circumstances to tailor interventions accordingly.
  • Utilizing DSM criteria to evaluate substance use diagnoses and determining the appropriate level of care based on ASAM guidelines.
  • Developing detailed substance abuse counseling components for each client’s treatment plan, ensuring they align with best practices.
  • Creating comprehensive discharge and continuing care plans, promoting smooth transitions for clients to the next level of support.
  • Maintaining meticulous documentation of all sessions, assessments, referrals, and follow-ups within client clinical records.
  • Managing a caseload, ensuring timely updates and communication for case management and treatment planning.
  • Coordinating support services for clients and their families while recording all relevant information within their clinical records.
  • Engaging with referral sources such as legal representatives or educational personnel, fostering a holistic support system for clients.
  • Facilitating interventions for clients during crises, utilizing a multidisciplinary approach to address immediate needs effectively.
Patient Population / Client Focus

This role focuses primarily on adolescents and young adults in school settings who may be experiencing mental health challenges or co-occurring disorders. Your expertise and empathetic approach will be crucial in crafting personalized treatment plans that not only address issues related to substance use but also consider other mental health conditions.

Qualifications
  • A Bachelor’s degree in Social Work or a related field, with a preference for candidates who hold a valid LCSW license.
  • Strong professionalism, excellent therapeutic boundaries, cultural competency, and self-motivation.
  • Exceptional organizational and time-management skills, able to maintain flexibility and adaptability in a fast-paced work environment.
Preferred Experience
  • Previous experience working in school settings or with youth populations is highly desirable.
  • Experience in providing co-occurring counseling and familiarity with integrated treatment approaches.
  • Proven track record in case management and navigating multidisciplinary teams.
