Key Responsibilities- Utilize your expertise to conduct comprehensive psychosocial assessments, formulating independent diagnoses based on established mental health frameworks.
- Partner closely with patients to develop personalized treatment plans, ensuring they reflect the unique needs and preferences of each individual.
- Implement evidence-based interventions, which may include solution-focused therapy, behavioral activation, and motivational interviewing, helping clients navigate their treatment journey.
- Collaborate with a multidisciplinary team, including psychologists and primary care providers, to create integrated care pathways that support comprehensive patient well-being.
- Track patient progress systematically through outcomes measurement and monitoring, utilizing both in-person and virtual consultations to adapt treatment as needed.
- Assist in coordinating psychotropic medication management in collaboration with prescribing medical providers, including adherence tracking, side effect management, and treatment effectiveness assessments.
As a key member of our clinical team, you will serve a diverse population, ranging from children and adolescents to adults facing various mental health challenges. Your efforts will support individuals dealing with anxiety, depression, trauma, and relationship issues. Understanding the cultural context of our clients is essential, as it informs the therapeutic approaches and interventions you will employ.
Qualifications- A Master’s degree in Social Work from an accredited program.
- A current and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in California.
- A minimum of one year of experience as an independently licensed therapist, demonstrating a strong foundation in mental health counseling.
- Experience utilizing digital health tools for teletherapy sessions, enhancing client engagement and broadening access to care.
- Familiarity with evidence-based treatments for specific populations, particularly within community health settings.
- Demonstrated ability to work collaboratively within a healthcare team to support integrated care models.
