Key Responsibilities

• Conduct thorough biopsychosocial assessments tailored to the diverse cultural backgrounds of clients, addressing their mental health, substance use, and medical needs within a primary care framework.
• Deliver behavioral health services to individuals, couples, and families, including counseling, intake assessments, diagnosing conditions, treatment planning, and crisis intervention, ensuring comprehensive care.
• Document all client interactions, including mental health evaluations and progress notes, through our electronic health record system, guaranteeing accuracy and compliance with relevant regulations.
• Provide vital liaison services, linking clients to local resources and community agencies, particularly in accessing medication assistance programs and other supportive services.
• Collaborate with Quality Improvement teams to develop and implement effective processes for data collection, enhancing service delivery and patient care outcomes.

Patient Population / Client Focus

You will primarily serve a diverse clientele, including individuals with mental health challenges, substance abuse concerns, and complex social needs. Our clinic is focused on delivering culturally competent care to all age groups, ensuring that services are relevant and accessible. You will play a key role in addressing the unique challenges faced by the communities we serve, particularly in Hesperia and surrounding areas.

Qualifications

• Active and valid Licensed Clinical Social Worker (LCSW) license in the state of California.
• A Master's degree in Social Work from an accredited program.
• A minimum of three years of post-licensure experience in a clinical setting.
• Proven knowledge of various counseling techniques and crisis intervention methodologies, coupled with the ability to make independent judgments in assessing and delivering counseling services.

Preferred Experience

• Experience in group facilitation and leading therapeutic processes.
• Familiarity with electronic health record systems and telehealth platforms.
• Background in collaborating with interprofessional teams to streamline care approaches.

Compensation and Benefits
