Responsibilities:
Working under minimal supervision, utilizes professional standards to ensure safe, quality coordination and provision of social services to patients in pediatric, adolescent, adult, and geriatric age groups in the acute care setting while maintaining compliance with regulatory requirements, HIPAA, and NKDH policies and procedures.

Provides screening assessments of identified individuals in order to assist with resource management, care transitions, crisis management and interventions connecting patients and families with appropriate resources

Participates in rotational call

Collaborates to ensure optimal quality of care through timely coordination of health care services from admission through discharge with members of the health care team both within the organization as well as with community agencies

Works closely with healthcare team to create transition of care plans and maximize efficiency of care

Effectively utilizes organizational EMR

Facilitates support groups as appropriate

Actively participates in quality initiatives

Expedites access to the most appropriate level of behavioral health care for persons presenting with depression, anxiety, drug addiction and/or other psychological symptoms including risk of suicide and homicide

Facilitates nursing home placement

Initiates Adult and Child Protective Services reports

Provides information to patients, families and staff about community resources

Assist persons who are experiencing a variety of acute financial/support needs in the attainment of needed resources while taking into consideration financial, social and clinical factors.

May be asked to assist with patient care in any area of the hospital, as needed.

Qualifications:
Baccalaureate degree in social work (BSW) from an institution accredited by the CSWE, Registered Nurse, or education and training in psychology, sociology, or other field related to social work.

Required:

In-depth understanding of the age-related behaviors and their needs based on knowledge of the medical diagnoses of the patients served in order to develop and provide an individualized plan of care

Excellent interpersonal, communication and prioritization skills

Initiative and ability to make effective decisions within the framework of established policies and procedures

Possesses good problem solving abilities including follow-up and reporting

Calmly and confidently handles highly stressful situations

Energetic and motivated

Ability to effectively work closely with interdisciplinary team members, law enforcement, and judiciaries to coordinate appropriate plan of care with safety of patient, staff, and community a priority

Knowledge of Medicare, State, and regulatory agency rules and standards related to hospital

Written communication skills as well as computer skills for documentation in the medical record
Demonstrates accountability for own actions, understands own limitations, seeks advice and assistance when necessary, and assume responsibility for personal growth and skill development
