Position Summary
Obtains, reviews and processes referrals for admission to skilled nursing facility
Responsibilities:
Develops collaborative relationships with hospitals, physicians, community organizations, team members and other referral sources.
Analyzes referrals to determine if the patient meets admission criteria both medically and financially.
Verifies insurance coverage and obtains authorization.
Coordinates the patient’s discharge from the hospital, home, or other setting and facilitates admission to skilled nursing facility.
Prioritizes potential admissions based on the patient’s medical needs, financial support and discharge potential.
Meets with residents/families to explain the admission’s agreement and obtain signatures.
Enters referral information into the electronic referral log.
Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
Maintains compliance with all Orlando Health policies and procedures.
Other Related Functions
Represents skilled nursing facility at community events.
Qualifications:
Education/Training
High school diploma or equivalent
Licensure/Certification
None
Experience
One (1) year of experience reviewing and processing referrals for admission to medical facility.
