Incumbent functions as both a positive role model and advocate for a youth in his/her family system and community during the Wraparound process. They are responsible for helping the youth identify their unique strengths and needs and voice their own perspective.  They also ensure that other team members are able to understand that perspective.  Incumbent represents youth orientation and supports a youth perspective in all internal and external agency activities.  As a member of the Wraparound Team, they support building confidence and improving self-efficacy with the youth and their family so they can maintain progress after formal Wraparound has ended.

 

For internal candidates/union positions only: Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.

Responsibilities:
Universal Job Skills:

Communicate role, responsibility and agency philosophy in order to assure mutual respect, confidence and trust with the youth and family and other stakeholders.

Provide non-judgmental and unconditional support to the youth and family.

Assess for immediate safety and stabilization needs.

Customize helping approaches to fit the family’s uniqueness, personality, culture and interest.

Report relevant information to the right people at the right time.

Maintain a focus on strengths, needs and creative solutions.

Utilize the family’s expertise in problem solving and solution seeking.

Work interdependently with others toward common goals.

Respond to youth or family needs in a timely fashion.

Complete required documentation within established time-frames.

Instill hope for the future by communicating and behaving with confidence and reassurance. 

Welcome, Engagement and Assessment:

Meet with and engage the youth, communicating compassion, support, respect and enthusiasm for them in the Youth Peer Support Partner’s role.

If appropriate, share personal experience as a consumer of social services that will assist in building rapport and trust with the youth and family.

Listen actively and support and encourage youth to identify unique strengths, needs, interests and perspective.

Reassure and communicate hopefulness and clarify information to youth about the Wraparound process. 

Individualized Service Planning:  Support youth and his/her perspective in Wraparound Team meetings.

Encourage youth to take an active role in their care planning.

Work to ensure the team environment feels safe for the youth.

Establish and maintain positive working relationships with all team members, especially family members.

Consult with Care Coordinators, Family Support Partners and Supervisors on issues related to supporting and strengthening youth/family/community connections.

Review the Wraparound Plan and make suggestions for improvement/ modifications that would improve youth participation.

Help to promote a culture that sees youth participation as valuable.

Implementing Services: Provide a range of supportive/helpful interventions and activities as agreed to with the youth and documented in the Wraparound Plan.

Adapt activities and interventions to enhance focus on strengths, needs and creativity without changing the basic nature of agreements and plan.

Assist the youth in analyzing progress toward vision and goals and encourage their feedback to their team.

Communicate ideas by using own life as a learning and teaching tool (when helpful to family and plan).

Role model, teach and coach youth in skill development identified in the Wraparound Plan or as age/developmentally appropriate.

Provide information to youth and team about available community resources and programs.

Communicate progress and concerns to the appropriate team members.

Participate in continuous improvement efforts by sharing perspectives in agency activities such as Quality Improvement reviews, family staffing or by other means.

Transitioning Services to Natural and Other Community Supports:Serve as a valuable link to ongoing needed services, resources or natural supports during the transition process.

Offer encouragement and support to youth and family during the transition planning process.

Reach agreement with the youth about post-transition contact.

 

Access to and protection of PHI:

Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).

Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).

 

Other duties as assigned.

Qualifications:
MINIMUM:

Personal experience as a consumer of mental health services.

High school diploma/GED AND Eligible to become peer counselor certified (See WAC (phone number removed) OR Certified Peer Counselor.

Ability and willingness to obtain WA State Agency Affiliated Counselor Certification.

1 year formal/paid work experience.

1 year of work or volunteer experience in a responsible position working with youth or families.

Representative of the youth perspective and culture.

Must have a valid driver’s license, proof of current automobile insurance and reliable transportation.

Must have acceptable driving record (Abstract-Employer Version (Full Record).

Availability to work flexible hours (early evenings, nights, weekends).

Excellent written and oral communication skills.

Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.

Support and contribute to a creative, collaborative and respectful environment that promotes teamwork.

Criminal History Background Checks are required prior to employment.
