Corus Health is the proud recent recipient of Albuquerque's prestigious Top Workplace award As a full-service hospice and home care agency providing a broad range of services to our customers, we are currently seeking a Hospice Bereavement Coordinator to oversee training, scheduling, and all support services associated with our Hospice Volunteers. As a member of the hospice interdisciplinary team the Bereavement Coordinator is responsible for the provision of bereavement support to patient families, facility personnel, and Corus Health employees. The Bereavement Coordinator will also serve in an overflow capacity, assisting the chaplain staff in the provision of spiritual support to patients & families, in keeping with their individual desires and belief systems. Bereavement Duties & Responsibilities:

Provide planning and oversight for all functions of the bereavement support programs.

Coordinate with chaplain staff regarding pre-death bereavement assessments.

Maintain communication link with chaplain staff regarding the potential bereavement needs of patient families as presented during patient conferences.

Complete post-death bereavement assessments/care plans.

Assume responsibility for evening, weekends, and holidays on call duty as scheduled and consistent with the policies and procedures of agency.

Develop and maintain a resource list of community and church-sponsored grief support groups and activities.

Provide bereavement follow-up services as defined by Corus Hospice procedures, including sending condolence cards from IDT, regularly scheduled Bereavement support mailings, conducting bereavement counseling through phone and home visits as requested, maintain proper records of bereavement services provided, conducting biannual grief support events and Remembrance Services during the holidays.

Develop and track statistical information regarding utilization of services and feedback from bereavement services and events. Implement and analyze satisfaction survey regarding bereavement services at the end of bereavement support period.

Screen potential volunteers interested in bereavement care; provide specialized training for bereavement support.

Within considerations such as priorities, perceived need, and time allotment, conduct additional activities as approved by the Hospice Director, such as agency-sponsored support groups, specialized bereavement support events, and in-service education.

Plan for Bereavement Program development, refinement, and on-going setting of priorities through annual evaluation of services and review with the Clinical Manager.

Serve as resource to patient families, facilities, and Corus staff regarding grief and loss.

Chaplain Duties & Responsibilities:

Provide direct spiritual support to patients/families.

Make contact with clergy or appropriate representatives of patient/families as indicated.

Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated.

Participate in patient conference by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated.

Maintain proper records of visits to patients/families.

Conduct or make arrangements for funeral or memorial services per patient family request.

Work with staff, clergy, health care professionals and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.

Assume responsibility for evening, weekends, and holidays on call duty as scheduled and consistent with the policies and procedures of agency.

Demonstrated knowledge of and comfort with diverse ethnic and cultural beliefs/practices.

Thorough understanding of grief process (normal/abnormal, anticipatory, bereavement).

Proficient in counseling of the dying patient and bereaved.

Accountability in job performance and for improving job knowledge and skills.

Support agency’s philosophy, goals, objectives, mission, vision and values.

Requirements

Qualifications & Requirements:

Complete understanding of Continuing Quality Improvement (CQI) process

Demonstrated knowledge of the grieving process

Ability to work effectively with a diverse ethnic and cultural population

Understanding of financial issues and community resources which may assist patient

Bereavement experience in varied multi-disciplinary settings

Minimum two years experience in health or related field required

Valid New Mexico driver's license and insured, reliable transportation

Accepts other assignments as appropriate
