Summary:
Under general direction of leadership, the Hospice Supervisor RN position is responsible for supervising clinical operations of the in-home hospice program. This position has 24-hour responsibility and accountability for planning and coordinating safe staffing and to ensure optimal end of life care provided by the hospice program. The Supervisor RN will lead nursing and aide activities to accomplish the goals of the organization by means of motivating, educating, guiding and communicating. This position works under the Operations Manager. This position links nursing leadership with nursing care through the application of organizational and nursing service policies, procedures, and care delivery model. The Supervisor RN uses the Code of Ethics for Nurses with Interpretive Statements to guide leadership and practice. The Supervisor RN attains knowledge and competence reflective of current nursing practice; promotes innovation and futuristic thinking by incorporating evidence-based practice and research findings.

Responsibilities:
(30%)* Supervises and coordinates all in-home hospice patient care activity, including on-going evaluation, treatment, transfers, etc. Provides on-going evaluation of patient status and suggests changes in treatment program to physicians involved as necessary. Interprets physician orders and helps staff work within orders to obtain desired result. Remains available for staff consultations and recommends changes when appropriate.

(25%)* Responsible for the quality of patient care. Responsible for the general oversight of hospice patients and their support network. Collaborate with hospice providers, social workers, volunteers and spiritual and bereavement specialists to coordinate quality end of life care.

(25%)* Determines staffing needs and scheduling patterns for coverage of program. Determines and maintains the staffing and schedule patterns to provide 24 hour/day, 365 days/year coverage. Approves or denies requests for vacation, personal holidays, or sick days and corrects staffing accordingly. Makes changes in staffing assignments as level of activity changes. Participates in direct patient care as needed which includes all shifts and days of the week.

(20%)* Assures compliance with all policies, procedures, regulations, standards and laws such as but not limited to Joint Commission, Centers for Medicare and Medicaid Services (CMS) and Michigan Department of Public Health (MDPH)

 

OTHER DUTIES AND RESPONSIBILITIES:

 

Assist to design and conduct Process Improvement and Standards of Care.

 

This includes identifying standards of care along with policy development and implementation.

 

Works with the Operations Manager to establish goals and objectives and possesses a high degree of clinical and end of life expertise.

 

Direct reports include hospice nurses.

 

Assists with hiring, disciplining, counseling and discharging employees per health system policies.

 

Counsels staff as needed. Review and follow up on all variances for in-home hospice.

 

Plans, develops, implements and evaluates educational programs for orientation, staff development, and continued growth of all employees within the department.

 

Evaluates staff within area of responsibility through annual Job Performance Reviews.

 

Participation in the patient’s coordination of care- Ongoing interdisciplinary assessment of the patient.

 

Development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s)- Providing services that are ordered by the physician as indicated in the plan of care- Patient, caregiver, and family counseling- Patient and caregiver education- Preparing clinical notes- Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to current plan of care.

 

Proactively works with patients and visitors to ensure customer satisfaction, address complaints/concerns in order to meet their individual needs.

 

Problem solves, evaluates and decides on course of action and alerts additional resources as needed.

 

Takes an active part in nursing management meetings.

 

Participation in the organization’s quality assessment and performance Improvement (QAPI).

 

Participates on or leads nursing service committees as appointed by Operations Manager. 

Certifications and Licensures:
BLS - Basic Life Support Required Within 1 month of hire or transfer.

 

CHPN - Cert Hospice & Palliative Nurs Preferred

 

Hospice and Palliative Care certification preferred.

 

FINGERPRIN - Fingerprinting Required 

 

RN - Registered Nurse Required

 

TEAM ADVANCED - Techniques for Effective Aggression Mgt Required 

Required Education:
Bachelors Degree

Other Information:
EXPERIENCE, TRAINING AND SKILLS:

 

Two years clinical experience working in the medical center or similar setting.

 

Hospice experience preferred.

 

Leadership skills needed to effectively direct a group.

 

Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.

 

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

 

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Medium. Must be able to occasionally (0-33% of the workday) lift or carry 21-50 lbs., frequently 34-66% of the workday 20 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls and constantly (67-100% or the workday) 10 lbs.
