Summary:
Responsible for receiving and processing incoming inquiries and referrals regarding home care services and equipment. 

Responsibilities:
(25%)* Upon receipt of referral from the medical center’s Home Care Coordinator/Discharge Planner, visits acute care patients/families to explain services and complete referral information and download to Agency.

 

(25%)* Provides patients/caregivers/physicians with the necessary information to promote informed decision-making leading to the achievement of optimal level of independence.

 

(25%)* Assists in determining if referral is appropriate and eligible for services.

 

(25%)* Coordinates the HomeCare plan with medical center’s case manager, discharge planner, physician, ancillary staff, patient, family, and other community resources as appropriate.

 

OTHER DUTIES AND RESPONSIBILITIES:

Ensures that orders and all necessary information to initiate services are obtained.

 

Responds to inquiries and requests for client services, providing information regarding agency and community services available.

 

Performs pre-admission calls.

 

Receives and processes physician orders.

 

Communicates patient needs with agency personnel; changes in orders.

 

Triage patient calls to appropriate agency personnel 

 

Utilizes Agency documentation system to record services provided according to agency, professional, and program standards. 

 

Assists in maintenance of patient records.

 

Educates external health care professionals regarding range of services agency provides.  Assists in Agency community education activities as requested.

 

Participates in Agency quality and performance improvement process.

 

Participates in Agency projects and committees as assigned.

 

Participates in weekend and vacation rotations and on-call duties as identified in personnel policies.

 

Participates in staff meetings, in-service education programs, and other opportunities for learning.

 

Other duties as assigned.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

 

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

 

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

 

Overall vision and hearing is necessary with or without assisted device(s).

 

Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.

 

Some exposure to blood borne pathogens and other potentially infectious material.  Must follow MyMichigan Health bloodborne pathogen and TB testing as required.

 

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

 

Overall dexterity is required including handling, reaching, grasping, fingering and feeling.  May require repetition of these movements on a regular to frequent basis.

 

Physical Demand Level: Light.  Must be able to occasionally (0-33% of the workday) lift or carry 11-20 lbs., frequently (34-66% of the workday) 10 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.
