Responsibilities:
Perform various duties to provide quality care to residents to maintain the highest level of care as described below:

Assists in the evaluation and assessment of newly admitted residents as appropriate including performing and documenting admission assessments and entering medication orders in the electronic record. Created Baseline Care Plans and update as required.

Monitors residents for weight loss, alteration in ability to transfer or ambulate, change in ADL status, etc. Develops appropriate interventions to prevent decline.

Conducts resident rounds on a daily basis and as needed.

Directs and supervises nursing care delivered during shift.

Acts as a liaison between the physician, family, and other departments in the community.

Administers medications and treatments to residents as necessary. Acts as liaison to pharmacy to procure necessary medications when appropriate.

Observes, assesses, and documents changes in residents’ conditions and notify physician, resident’s representative, and appropriate nursing personnel, as appropriate.

Notifies the Abuse Coordinator (Executive Director / Administrator) immediately of all allegations or observations of all types of abuse, including resident to resident abuse.

Endeavors to maintain compliance with federal, state, local, and corporate regulations related to resident care and safety and staff conduct.

Maintains current informative clinical records based on current standards of practice and regulations.

Completes all required documentation accurately and timely following all community policies and procedures.

Directs and lead the day-to-day functions of nursing personnel.

Supervises nurses and nursing assistants on shift in documentation and work assignments.

Supervises or participates in change of shift reports as appropriate.

Communicates and interacts effectively and tactfully with residents, staff, volunteers, and families to ensure residents are receiving the best quality care.

Promoted resident safety through supervision and oversight of residents and staff in adherence with all community safety protocols.

Responds to emergencies. Assesses resident condition, determine medical attention, and call appropriate services, as needed.  Informs family of transfer or admission to hospital.

Completes incident reports and follow-up documentation following an incident or accident in adherence with the community risk management protocols.

Attends all in-services and training activities as required.

Attends and participate in continuing education programs designed to keep abreast of changes in the profession.

Demonstrates consistent management of staff and resources.

Participates in the interviewing, discipline, and performance evaluations of staff, understanding that final authority belongs to the Director of Nursing.

Participates in developing, planning, conducting, and scheduling in-service training classes to help ensure a well-educated nursing staff.

Assists in orientation for new employees to their duties according to the corporate guidelines. Nurtures a culture of friendliness and helpfulness toward new employees.

Supervises proper care and use of equipment and waste disposal procedures in accordance with community policies.

Follows the designated dress code being mindful of proper footwear, hygiene and cleanliness.

Fosters self-respect and a feeling of worth in each resident by consistent kindness, understanding, and patience in their care.

Organizes and maintains all records necessary.

Uses proper care and use of equipment and waste disposal procedures in accordance with corporate policies and procedures. Reports equipment repair and maintenance to direct supervisor.

Observes all community policies and procedures, including but not limited to safety, infection control, residents’ rights, and those contained in the employee handbook and nursing policy and procedure manual. Complies with all sanitation, personal hygiene and health standards.

Ensures quality customer service to the residents, families, and staff the department serves.

Follows all written and oral directions as given by supervisor.

Completes all required documentation accurately and timely following all community policies and procedures.

Follows all departmental safety rules and utilize proper body mechanics in the performance of all tasks.

Participates in all in-services and activities as required.

Maintains the confidentiality of all protected health information whether electronic, written, or oral exposed during the course of assigned duties or as a result of an incidental disclosure. Follow community policies in all matters related to residents’ protected health information (HIPAA).

Performs all other duties as noted on accompanying Task List and as directed by the Director of Nursing. Follows all written and oral directions given by supervisors.

 

Qualifications:
Graduate from an approved school of nursing required.

Current LPN state license required.

CPR Certification required.

Competent in basic nursing skills including adept resident assessment.

Knowledge of drug interactions and side effects.

Proficient in reading, writing, grammar, and mathematics skills.

Possess the ability to communicate effectively and deal tactfully with personnel, residents, families, visitors, government agencies, and the general public.

Display leadership qualities, good communication skills, and a desire to continuously learn.

Come to work as scheduled and consistently demonstrate dependability and punctuality.

Ability to follow written and oral directions.

Ability to work independently.
