Responsibilities:
 

 

MAJOR DUTIES

Serves as a clinical pharmacist in the Pharmacy Service of the MEDDAC. Shares clinical pharmacy duties and responsibilities with the Clinical Pharmacy Coordinator. Works within the privileges approved by the hospital Credentials Committee. More specifically performs the following:

 

1. Gathers, interprets and analyzes patient-specific data, assesses medical problems, evaluates current drug therapy, develops therapeutic plans, monitors drug therapy, dispenses and administers medications as needed, and consults with other patient care providers to improve patient outcomes. Decides whether non-formulary medications are needed to reach therapeutic goals.

2. Conducts necessary patient and family-centered education related to the medication treatment plan, expected outcomes (goals of therapy), potential adverse effects, and planned follow-up and monitoring.

3. Participates in or conducts studies, surveys or pilot evaluation projects involving pharmaceuticals. Designs and conducts studies intended to evaluate drug efficacy/effectiveness, safety and economic impact for the conditions being treated. Conducts medication use evaluation and develops evidence-based prescribing guidelines to assist providers in ensuring appropriate/safe medication use. Where improvement in these areas can be identified, counsels or recommends alternatives that will satisfy the medical needs of the situation. Looks for indications of misuse or abuse of pharmaceuticals and recommends corrective action. As tasked, provides pharmacy input to quality improvement reviews and similar processes.

4. Conducts training of other medical professionals, pharmacy students/interns, and non-professional staff on new developments in drug therapy (e.g. advises on the effectiveness and safety of various treatment regimens and alternatives), standards of care and cost-effective prescribing. Trains patients and nonmedical attendants in the preparation, storage, handling and administration of medications when appropriate.

Qualifications:

 Basic Requirements for Pharmacist:

Education: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Exemption: You are exempt from the basic requirement above if you are a current federal employee occupying a position in the 0660 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.

Licensure: A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia.

Medical Requirement: Must be able to distinguish basic colors.

Experience: One year of professional pharmacy experience which includes preparing pharmaceuticals, providing medication counseling to patients, and providing pharmaceutical education to healthcare professionals.
