Overview:
We are seeking a hand therapist to be a part of the M Health Fairview Rehabilitation team at M Health Fairview Clinic - Blaine The orthopedic suite on the clinic's second floor houses sports medicine and orthopedics, pain management, orthotics, podiatry, hand therapy, and physical therapy. This position is a fantastic opportunity to build relationships with 7-9 therapists and referring physicians onsite daily. The therapist is responsible for providing safe and effective delivery of patient care including examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

0.6 FTE position, authorized for 48 hours per pay period

Schedule: Monday, Wednesday and Thursday

M Health Fairview Rehabilitation offers a range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Rehab’s continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

Responsibilities/Job Description:
The therapist partners with the patient and their care providers while performing evaluation and re-evaluation.

 

Develops an individualized plan of care through coordination

Captures and applies pertinent data, performs examination, and resolves treatment diagnosis

Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals

Implements and modifies care plan based on reassessment and patient response

Coordinates care and communicates optimally with interdisciplinary team for all aspects care & establishes appropriate discharge plan

 

Partners with care team to provide skilled intervention

Implements and alters treatment to reflect change in patient status and response to treatment

Assesses patient and/or caregiver learning needs and provides appropriate education

 

Documentation/Billing

Uses acceptable medical terminology and follows professional practice standards.

Completes documentation in a timely manner per department standards

Stays in sync with department billing standards

 

Professional Expectations

Prioritizes tasks and patient care for effective time management.

Understands and follows professional licensure/certification requirements

Advises team members and/or supervises students as designated

Pursues professional development to support clinical standard processes

 

Quality Improvement

Crafting and implementing improvements, and participates in and supports entity/system initiatives

Qualifications:
Required 

Occupational Therapy Licensure in State of Minnesota or temporary license
Active Minnesota Basic Life Support Certification

 

Preferred 

3+ year(s) of Hand Therapy experience 

Master’s or Doctoral Degree in Occupational Therapy
Certification with the National Board for Certification in Occupational Therapy
Certified Hand Therapist (CHT)
