Overview:
The Case Manager will be responsible for providing case management to patients including discharge planning, family education, referral source development and management, and insurance utilization review for patients on the unit.  The Infection Prevention Nurse is responsible for the facility Infection Prevention and Control Program (IPCP), which is designed  to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections. 

 

SAFETY SENSITIVE POSITION:

 

This position is a designated as “Safety Sensitive Position” under Act 593 of the State of Arkansas. An employee who is under the influence of Marijuana constitutes a threat to patients/customers which Conway Regional is responsible for in providing and supporting the delivery health care related services.  

Qualifications:
Registered Nurse with 3-5 years experience in a clinical practice

Bachelor Degree in Nursing or health related field is preferred

Maintain a current, unrestricted Arkansas RN license

Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications are required.

Be qualified by education, training, experience, or certification in infection control.

Excellent written and verbal communication skills to include computer knowledge

Compliance with the requirements stated in the RN Job Description

Responsibilities:
Case Manager:

Serve as a liaison between patients, their families, physicians, and other health care personnel.

Responsible for Discharge Planning of all Acute Care patients

Conducts Discharge Planning Meetings per Arkansas Department of Health (ADH) Rules and Regulations

Collaborates with Utilization review of all Acute Care patient services.

Participates in appropriate department/hospital committees and meetings.

Investigates and resolves complaints or refers problems to appropriate Manager/Director

Maintains positive relationships with other Case Managers, and contacts at area facilities.

Develops and implements QI data. Reports data monthly to leadership and quality council.

Conducts relevant in-service training for department personnel.

Infection Preventionist:

Ability to follow standard transmission-based precautions to prevent the spread of infections.

Collaborate with the facility leadership and the medical director to develop, implement, and evaluate the annual infection prevention goals.

Establish a facility-wide system for the prevention, identification, investigation, and control of infections of staff, patients, and visitors.

Conduct outbreak investigations.

Assess the need to develop in-service education for individual departments, general orientation, and annual review as needed: education includes but not limited to: Hand hygiene

Cleaning, disinfection, and sterilization

Specific direct and indirect care settings

Therapeutic and diagnostic procedures and devices

Use of isolation/barrier precautions when indicated.

Patient placement, transfer, and discharge

Environmental hazard

Use of patient care products and medical equipment

Staff immunization programs

Assess the need for, develop, and implement written policies and procedures for infection control.

Enforce policies prohibiting staff with communicable diseases or infected skin lesions from direct contact with patients and their food.

Enforce hand hygiene policies consistent with accepted standards of practice.

Be accountable for surveillance of healthcare-acquired and community-acquired infections for staff, and visitors.

Maintain current knowledge of federal, state, and local regulations and ensure that the facility leaders are informed of appropriate issues; understand and comply with infection control, safety, and OSHA procedures and regulations.

Implements and administers the employee health program for DRMC.

Coordinates policies & planning for the employee health program with hospital leadership, infection prevention, pharmacy, other departments and relative external agencies.

Conducts health inventories for new employees, including immunization status or history of vaccine preventable diseases & history of conditions that may predispose personnel toward acquiring or transmitting infectious diseases as well as determines prospective employee’s ability to perform the essential job functions.

Coordinates all employee health required vaccinations, immunizations, & post exposure prophylaxis

Conducts personnel health assessments on an as needed basis as required to evaluate work related illness or exposures to infectious diseases.

Investigate, assess & follow up on work related injuries & illnesses. Assist employees in obtaining medical care as needed for work related illnesses or injury.

Maintains a record on health care personnel that includes information obtained during the health screening, immunization records, results of tests obtained in any screening or control programs, and reports of work-related illnesses, injuries or exposures in accordance with state and federal regulatory requirements.

Ensures that Employee Health policies are current and revised annually.

Adheres to local, state and federal regulations pertaining to Employee Health

 

Other duties and responsibilities as assigned by management. 

Participates in weekend rotation of call for nursing.
