The Transitions in Care Manger is responsible for providing leadership and direction in transitions in care throughout the Quad-City Region. This will assure coordination of patient services among the multidisciplinary team of health care providers throughout the continuum of care. Collaborating with community providers including but not limited to-provider clinics, extended care facilities, home care, behavioral health acute care and community agencies such as Alternatives for the Older Adult. Will lead ad guide navigators using evidence based best practices such as Integrated Chronic Care Disease Management ICCCDM.  Focus population will include Accountable Care participates in the Quad City region. The Transitions in Care manger is responsible for developing a baseline education related to care coordination at eth various levels of care in the continuum.

Responsibilities:
Care Coordination 

Perform as an expert within care transitions with focus on high risk population.  

Uses evidence based practices to implement motivational interviewing 

Collaborates with referring facilities, community service providers, and other agencies and support groups 

Collaborate with the care team to develop patient centered goals 

Provide leadership and care coordination best practices to reduce readmissions 

Quality 

Oversight, implementation, and evaluation of population-specific  performance improvement initiatives, quality metrics, and core measures 

Partners with physicians and patient care team members to achieve progress toward outcomes 

Supports competency development and implementation with the staff providing care to the identified patient populations serviced 

Engages in innovative care coordination to provide safe care progression for all populations, especially the complex and fragile. 

Leadership 

Mentor all team members that have a role in transitions in care 

Models behaviors for demonstrating values in performing job responsibilities. 

Provides leadership to improve performance, based on process improvement methodology and reports short and long term outcomes. 

Monitors and reports achievement of key strategic and operational goals to management team, providers, and staff 

Manages to budget and expense responsibility for the Case Management 

Creates an environment conducive to the development of clinical competence, self-confidence, professional growth, and job satisfaction for direct reports.   

Provides specific and timely feedback to employees to recognize positive demonstration of values and correct performance that does not reflect values. 

Provides mentoring to key team members demonstrating interest and talent to become future leaders. 

Ensures competent management staff and plans for continued professional development 

Qualifications:
Education:

Completion of a BSN or higher nursing program.

Experience:

Preferred 5 years in case management or similar role.

License(s)/Certification(s):

Current licensure in good standing to practice as a Registered Nurse in IL and IA 

Valid driver’s license when driving any vehicle for work-related reasons. 

Knowledge/Skills/Abilities:

Comfortable with computer—keyboard and application skills.   

Basic knowledge of Excel.   

Writes, reads, comprehends and speaks fluent English.
