- Monday thru Friday ***This is a fast-paced, multiple location shift helping folks get ready for the day, administering medications, and driving individuals to day service. Must have valid driver's license and valid personal care insurance. Will drive company car the majority of shifts.

About the Role
Thriving Together Support Services in Greenfield, IN is looking for compassionate and dedicated Home and Community Support Professionals (HCSPs) to join our growing team In this rewarding role, you'll make a meaningful difference in the lives of individuals who need support, helping them thrive in their daily lives.

Responsibilities:
• Provide direct care and personalized support to individuals with disabilities or special needs
• Assist clients with daily living activities including personal hygiene, meal preparation, and mobility
• Implement individualized support plans to promote independence and quality of life
• Transport and accompany clients to appointments, community outings, and activities
• Document care activities, observations, and progress accurately and in a timely manner
• Communicate effectively with clients, families, and care team members
• Foster a safe, supportive, and positive environment for all individuals served

Requirements:
• Previous experience as a DSP, HCSP, caregiver, or in a direct care role preferred
• Genuine compassion, patience, and dedication to supporting others
• Ability to pass a background check and meet state compliance requirements
• Valid driver's license and reliable transportation preferred
• Strong communication and interpersonal skills
• Ability to work independently as well as collaboratively with a care team
• Flexibility to work various shifts including evenings, weekends, or holidays as needed
