Qualifications
The Primary Care Nurse Practitioner provides direct care and education to Health Center patients and ensures continuity and quality of care. Practitioner will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.
Responsibilities:
- Provide direct patient care, counseling, and teaching to our primary care population in an ambulatory setting. Care for both acute and chronic conditions for new and established patients.
- Perform complete histories and physical examinations.
- Order, interpret, and evaluate appropriate laboratory and diagnostic tests.
- Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
- Order medications and write prescriptions according to organizational and regulatory policies and procedures, including agreement with collaborating physician as required
- Collaborate and consult with a multidisciplinary team of providers and other staff, including supervising physician, as appropriate.
- Demonstrate expert clinical judgment skills to function in an independent role.
Qualifications
- Graduate of Master's of Science in Nurse Practitioner program or have completed a Certified Nurse Practitioner program as well as Massachusetts licensure and Board Certification.
- Experience with Patient Centered Medical Home model and working with underserved populations
- Knowledge of and experience with EMR and Microsoft Office systems required, with Epic strongly preferred
