Field & Remote Location: McAllen, TX Pay Range: $40-48 Hourly, plus mileage reimbursement Home Health RN Care Manager Responsibilities: Conduct health screenings and risk assessments Identify special-needs members and determine eligibility and service needs Coordinate care with medical, behavioral, and social services staff Collaborate with community outreach teams and member advocates Educate providers and members on available program services Assist with care plan management, including working with caregivers, DFPS caseworkers, and physicians Follow up with members and providers by phone Complete assigned tasks and document outcomes, service satisfaction, and follow-up details Meet productivity expectations by completing weekly outreach for all assigned members Maintain accurate, clear, and concise documentation Home Health RN Care Manager Qualifications: Graduate of an accredited Nursing program Active RN (State or Compact License) or Nurse Practitioner License 4???6 years of related experience required Excellent computer skills Home health experience preferred If you are interested in this Home Health RN Care Manager position, please apply to this posting with Luke Heuser at A-Line
