Your primary role will be to coordinate discharge planning and assist residents and families in finding appropriate discharge placements. This full-time position operates on a Monday to Friday schedule. Job Summary As a social worker, you will: Provide supportive services and counseling on healthcare and home care programs. Collaborate with an interdisciplinary team to address the social, emotional, and economic concerns of patients, clients, residents, and their families. Offer crisis intervention and help families navigate complex medical situations. Coordinate healthcare programs among patients, families, and healthcare teams. Respond to suspected abuse, neglect, or violence in accordance with ethical guidelines and state laws. Develop care plans by connecting families with resources from social, health, and human services agencies. Participate in care conferences and transitional rounding as an advocate for patients. Demonstrate critical thinking, problem-solving, and effective communication skills. Qualifications To thrive in this role, you should have: A Bachelor’s degree in Social Work from an accredited program. Healthcare and/or mental health hospital experience (preferred). Basic Life Support (BLS) certification within six weeks of employment (if required). A valid social work license in good standing in the state(s) of practice.
