Your Role as a Home Health Nurse In this rewarding position, you will: Provide compassionate home-based care to patients recovering from illnesses, injuries, and chronic diseases. Collaborate with a team of physicians while overseeing LPNs, CNAs, and aides in delivering exceptional patient care. Conduct patient assessments, document progress, and communicate changes to physicians. Educate patients and their families on care plans, medications, and home safety. Ensure patient goals are met and coordinate discharge from services. What We’re Looking For To thrive in this role, you should have: An active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required). A minimum of one (1) year of clinical experience as a Registered Nurse. Current CPR Certification. Reliable transportation, a valid state driver's license, and current car insurance.
