Nurse Navigator - Yorba Linda Oncology - FT Days Position Summary: The Nurse Coordinator functions independently to coordinate care of oncological patients. The role includes planning, education, and support for oncology patients and their families, ensuring a complete continuum of care from pre?admit through hospitalization, discharge, and follow?up in the outpatient setting. The Coordinator maintains program data for analysis, participates in RFPs, and collaborates with the Director, Medical Director, and team for program development and growth. Nursing services are provided as needed, and other responsibilities may be assigned. Responsibilities: Co-ordinate and monitor inpatient and outpatient oncology care, ensuring seamless transitions across departments. Provide education and support to patients and families regarding cancer treatment plans and symptom management. Maintain and analyze program data, contributing to quality improvement and reporting. Assist in the development and growth of the oncology patient navigation program. Deliver nursing services as required and adapt to evolving clinical needs. Required Qualifications: BSN degree, or equivalent. Current California RN license and Basic Life Support certification. Minimum 6 months experience in an inpatient or outpatient oncology setting, or 1 year of clinical experience with a recent completion of a cancer basics class from the Oncology Nursing Society within the last year. Demonstrated customer?service skills appropriate to the role. Excellent written and verbal communication in English. Experience with and knowledge of chemotherapy agents. Ability to maintain an appropriate work pace for the workload. Preferred Qualifications: Knowledge of oncology/hematologic malignancy patient populations. Oncology Nurse Certification. Knowledge of complications associated with medical and surgical oncology conditions. Experience with immunosuppressive therapies. Bilingual skills to communicate effectively with patients and families.
