Job DescriptionWe are looking for a physical therapist wanting to be a Rehab Director. This position is at the VA in Jackson, LA.Key ResponsibilitiesPerform comprehensive evaluations of patients’ physical abilities, medical histories, and functional limitations.Develop individualized treatment plans based on clinical findings and patient goals.Administer therapeutic interventions including exercise programs, manual therapy, gait training, balance activities, and modalities (e.g., ultrasound, electrical stimulation, heat/cold therapy).Educate patients and caregivers on home exercise programs, injury prevention, safety, and physical wellness.Monitor and document patient progress, adjusting treatment plans as needed.Collaborate with physicians, nurses, occupational therapists, and other healthcare team members.Maintain accurate, timely clinical documentation in compliance with regulatory and facility standards.Ensure patient safety and uphold professional and ethical standards.Participate in discharge planning and provide recommendations for continued care or equipment needs.QualificationsDoctor of Physical Therapy (DPT) or master’s degree from an accredited physical therapy program.Current state licensure as a Physical Therapist (or eligibility to obtain).Strong communication, assessment, and critical-thinking skills.Ability to work independently and collaboratively within a multidisciplinary team.Preferred SkillsExperience in SNF.Familiarity with electronic medical records (EMR) systems.Working ConditionsStanding, walking, and lifting equipment or assisting patients regularly.
