As a Case Manager in our Rehabilitation program, you'll play a vital role in guiding patients and families through the healing process - ensuring each patient receives high-quality, personalized care that supports recovery, independence, and hope.Your responsibilities will include:Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rightsCoordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferencesDeveloping and implementing individualized treatment plans that reflect the patient's strengths, needs, and personal recovery goalsCompleting psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care teamCollaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered carePromoting HCA Healthcare's values of compassion, respect, and excellence through every patient and family interactionWhat qualifications you will need:The Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.Basic Cardiac Life Support(RN) Registered NurseBachelors DegreeHCA Florida Mercy Hospital is a 488-bed facility. We are accredited by the Joint Commission and offer a wide range of medical specialties. These specialties include emergency care, heart, and vascular care. We are designated by the Joint Commission as a Primary Stroke Center. Other specialties include maternity and neonatal care. We are a Level II NICU hospital. HCA Florida Mercy Hospital is CARF accredited for inpatient rehabilitation. Our facility offers a Comprehensive Community Cancer Program.
