Our Corporate Headquarters is in Hallandale Beach Florida.Our Mission Statement: We streamline patient care by ensuring real-time response, quality assurance, accurate medical decision-making, and thorough documentation, thereby improving both patient outcomes and facility efficiency.Position Overview: We are seeking a compassionate and experienced APRN to join our growing healthcare team, providing high-quality care in skilled nursing and long-term rehabilitation facilities. This role involves diagnosis, treatment, patient management, and interdisciplinary care coordination across a range of specialties, such as pulmonary and acute care, infectious disease, cardiovascular, pain management, psychiatry, and physical rehabilitation.Schedule: TBDKey Responsibilities:Deliver direct patient care in skilled nursing, rehab, and long-term care settingsPerform health assessments and physical examinationsDiagnose and develop treatment plans tailored to each patientPrescribe medications per Florida guidelines and clinical best practicesComplete documentation and maintain accurate health records (EMR/EHR)Conduct admission and discharge evaluationsCoordinate care with physicians, facility staff, and specialistsPromote preventive health practices and counsel patients on healthy livingEnsure continuity and quality of care with timely interventionsParticipate in facility rounds and contribute to reducing hospital readmissionsEducate patients and families about treatment plans and care routinesComply with infection control standards and regulatory policiesQualifications:Active Florida APRN licenseDEA registration (preferred)BLS and ACLS certificationsActive NPIMinimum of 3 years of nursing experience (Skilled Nursing preferred)Knowledge of post-acute care and geriatric best practicesStrong communication, assessment, and leadership skillsExperience with EMR/EHR systems (Point Click Care/ Matrix)Comfortable working independently in a fast-paced, clinical environmentSpecialties We re Hiring For:We are currently seeking APRNs with experience or interest in the following specialtyPulmonary & Acute CareIf you have a background in this specialty, we encourage you to apply and indicate your specialty interest in your application.If you're located near one of these areas or are open to relocation, we d love to hear from you.Preferred Skills & Knowledge:The ideal candidate should have experience or knowledge in the following areas:Performing physical exams and patient observationsBehavioral health assessmentSetting up and monitoring medical equipmentCommunicating test results to patients and their familiesRecording patient medical historiesFamily planning servicesPost-acute care experience (Preferred)Senior care practicesMedical-surgical nursing techniquesStrong anatomy knowledgeBenefits:Flexible scheduleMalpractice insurance providedContinuing education supportOpportunity to make a direct impact on patient outcomesA dedicated medical scribe for documentation support (Remote)Paid TrainingShadowingCompensation: Highly competitive, commensurate with experience and qualifications.
