This role offers the opportunity to provide meaningful, one-on-one care in patients' homes, helping individuals regain strength, mobility, and independence while maintaining autonomy in your practice.Join a mission-driven organization that values flexibility, professional autonomy, and collaboration within an interdisciplinary care team.Why You'll Love Working With UsFlexible scheduling with full-time or part-time options (.6 FTE or more)Generous paid time off to support work-life balanceComprehensive benefits including Medical, Dental, Vision, and Supplemental Insurance401(k) with company matchCompany-paid life insuranceTuition reimbursement and annual training budgetEmployee Assistance Program (EAP)Additional perks including legal services, ID protection, pet insurance, and financial wellness programsDeliver care where it matters most—at homeEnjoy professional autonomy with the support of a collaborative interdisciplinary teamEssential Duties and ResponsibilitiesConduct in-home physical therapy evaluations and develop individualized plans of careProvide skilled, evidence-based physical therapy treatments as ordered by the physicianAdjust treatment frequency and duration based on patient progress and reassessmentComplete and score OASIS assessments accurately and in compliance with Medicare regulationsMaintain knowledge of Medicare Conditions of Participation and payer requirementsPerform therapeutic interventions, including wound care when appropriateDocument all care and communication using point-of-care electronic systemsCoordinate daily schedules, participate in interdisciplinary care conferences, and support quality improvement initiativesSupervise Physical Therapist Assistants (PTAs) and CNAs as applicableManage patient care episodes including admissions, re-evaluations, and dischargesMaintain active Montana PT licensure, required continuing education, and ethical professional standardsParticipate in agency training and professional development activities REQUIREMENTSBachelor's degree or higher from an accredited Physical Therapy programCurrent Physical Therapist license in the State of Montana (or ability to obtain)Valid driver's license, reliable transportation, and appropriate auto insuranceAccess to high-speed internetTwo or more years of physical therapy experience preferredHome health, outpatient, or clinical rehabilitation experience a plusDepartment: Home HealthEmployment Type: Full-Time or Part-TimeSchedule: Monday–Friday, with flexibility based on FTE and patient care needsFull-time or part-time (.6 FTE or more) options available.
