We pride ourselves on hiring professionals who understand the unique dynamics of Home Health care.Qualifications:Graduate of an accredited Occupational Therapy Assistant programCurrent Texas state license as a COTATwo years of experience in Home Health preferredSpanish-speaking preferredResponsibilities:Adhere to established agency policies and proceduresProvide physician-prescribed occupational therapy services under a plan of care established by an Occupational Therapist (OT)Help patients improve or maintain their ability to perform activities of daily living (ADLs)Support patients in regaining independence and achieving their highest functional potentialCarry out skilled therapy procedures as ordered by the physician and directed by the OTCollaborate with the OT and other healthcare personnel in patient care planningReport changes in patient condition and progress to the supervising OTEducate patients and family members/caregivers on home programs and ADL strategiesParticipate in mandatory in-service and performance improvement activities as assignedAttend patient care conferences as scheduledDocument timely and accurate visit notes, progress updates, and care plan revisions as per agency requirements
