We invite you to become part of our integrated clinical team as a Licensed Clinical Psychologist. In this role, you will work onsite in long-term care facilities within your community, providing vital therapy, assessment, care planning, and behavioral health support. Your Responsibilities As a valuable member of our interdisciplinary team, your duties will include: Clinical Services Conduct comprehensive psychological evaluations, including diagnostic assessments and cognitive screenings. Provide individualized psychotherapy tailored to the needs of older adults and those with complex medical conditions. Facilitate family therapy and caregiver support sessions. Lead or co-lead group therapy sessions when appropriate. Implement behavioral management interventions to support residents, facility staff, and families. Develop and execute individualized treatment plans based on evidence-based practices. Manage your caseload independently with sound clinical judgment. Interdisciplinary Collaboration Collaborate daily with nursing staff, social workers, physicians, and rehabilitation teams. Participate in interdisciplinary meetings and care plan reviews. Serve as a behavioral health resource to facility leadership and staff. Provide consultation to facility teams regarding behavioral strategies and care recommendations. Documentation & Compliance Timely complete documentation using our Geriatric Notes Tool (GNT). Maintain accurate, comprehensive clinical records in compliance with regulatory requirements. Adhere to state laws, CMS expectations, HIPAA standards, and APA ethical guidelines. Facility Partnership Foster collaborative relationships with facility personnel.
