OverviewTamarack Health, Hayward Medical Center is seeking a compassionate and experienced leader to lead our Social Services team in supporting the social, emotional, and psychosocial needs of patients and residents in acute care, transitional care, and skilled nursing. This leadership role ensures the delivery of high-quality social services, effective discharge planning, and strong collaboration with interdisciplinary care teams.Key ResponsibilitiesCoordinate and oversee all aspects of social services operations to include staff leadership, budgetary practices, experience and quality practices. Lead transition of care initiatives including discharge, transfer, and home care planning in collaboration with physicians, nurses, and other care team members. Develop, implement, and evaluate programs, policies, and procedures related to social services in accordance with industry best practices and regulatory compliance. Collaborate with community and TH leaders and staff in building and maturing relationships that support patient, resident, and tenant wellbeing and continuity of care. Serve as an advocate and leader of patient and resident rights practices that ensure dignity and respect in all care interactions. QualificationsGraduate of a social work program; Master's degree in Social Work (MSW) preferred. Current Wisconsin Social Worker license required. Minimum of three (3) years of healthcare social work experience required. Prior leadership or management experience preferred. Strong communication, organizational, and interpersonal skills.
