We want your knowledge and expertiseJob Summary and QualificationsSeeking a Kidney  Transplant Coordinator who is a professional registered nurse that performs a variety of clinical services and patient care activities in collaboration with a multi-disciplinary healthcare team to ensure the smooth flow of the transplant evaluation and post-transplant care process. The coordinator works with patients and their families beginning with the evaluation for transplantation and continuing through and after transplantation in a compassionate and tactful manner in order to facilitate access to and provide continuity of care. Essential Job Functions: Assessments: Performs appropriate assessments to include psychological, physical, environmental and functional needs Education: Provides patient and family education specific to the patient’s disease process. Serves as a resource for education for patient, family, staff, hospital and community. Communication: Communicates with all members of the transplant team to include hospital staff and referring physician/staff Data Collection: Participates in the collection/entry of data in the patient medical record as required by all regulatory agencies Documentation: Documents assessments, interventions and follow-up accurately and timely Coordination of care through the phases of transplantation with the transplant team as follows:What qualifications you will need:Graduate of an accredited registered nursing program, BSN preferred Minimum 1 year experienceEmployee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable). Minimum License, Certificates, or Registrations Required: A valid Texas RN license; BCLSMethodist Hospital Specialty and Transplant has been nationally recognized for its record setting kidney, liver and pancreas transplant programs. It is home to the Texas Transplant Institute and is the largest living donor kidney transplant program in the US. According to the United Network of Organ Sharing database, the center reached a major milestone in 2019 by performing 231 living donor kidney transplants in a single year setting a new national record - that’s 65 more transplants than the second center on the list. In addition to transplant care, Methodist Hospital Specialty and Transplant is a full service hospital with over 260 beds that provides specialized services for liver cancer care, emergency care, weight loss surgery, urology surgery, vascular surgery, inpatient rehabilitation, colorectal surgery and behavioral health care.
