You’ll help patients set goals, navigate barriers, and access the resources they need to achieve lasting wellness.This is a great opportunity for someone who enjoys collaborative care, thrives on variety in their work, and is passionate about improving health outcomes through behavioral interventions and patient support. Key ResponsibilitiesProvide in-person and remote consultation to patients and primary care providers on a range of health concerns, including mental health, chronic conditions, and wellness managementAdminister and interpret screening tools, and assist patients with goal setting and readiness for change using motivational interviewingIdentify barriers to treatment or medication adherence and support patients in overcoming themFacilitate psycho-educational groups on self-management of health concernsConduct brief crisis interventions and safety planning as neededMaintain accurate client records, communicate regularly with healthcare team members, and provide referrals to additional behavioral health or social servicesCollaborate with Nurse Care Managers and other team members to address care gaps and quality metricsStay current on therapeutic techniques, best practices, and program admission/utilization criteriaProvide licensure supervision and perform utilization review activities as assignedAttend required trainings and maintain productivity expectationsRequirements, Skills, Knowledge and ExpertiseMaster’s degree in social work, marriage and family therapy, counseling, or psychology requiredQUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS • At least one (1) year of supervised experience treating mental health and behavioral issues related to chronic medical conditions required • Working knowledge xhmxlyz of DSM-5 preferredQUALIFICATION(S): LICENSURE/CERTIFICATION • Must hold Missouri licensure under one of the following: PLPC, PLMFT, LMSW, or full licensure in the state of MissouriYou’ll Be a Great Fit for This Role if You:Thrive in collaborative, multidisciplinary healthcare settingsEnjoy coaching and supporting patients to achieve meaningful, sustainable changesCommunicate clearly and compassionately with patients, families, and healthcare teamsAre adaptable, organized, and able to manage both scheduled and unscheduled patient needsBelieve in whole-person care and are committed to improving wellness for all individualsIND 2
