Position Description:

The Safety Net Case Manager is a fulltime, non-exempt, position that provides differential response, case management or concrete support services, and/or post reunification or after care case management services to families referred by the Child Welfare System, specifically Child Protective Services (CPS), to maintain the family unit and prevent or reduce the risk of families entering or re-entering the Child Welfare System. This position is primarily responsible for engaging families and connecting them with resources and services to build resilience again risk factors, learn skills to improve family dynamics, alleviate stressors within the family unit, etc. The Safety Net Case Manager works directly with Program Managers and Coordinators to ensure effective interventions for client cases and/or to address obstacles and barriers clients may be facing. Safety Net Case Managers will also provide a range of clerical and administrative duties which include but are not limited to covering the reception area, greeting office visitors, handling incoming communications, managing files, updating paperwork and other documents, perform other general office and clerical duties. Safety Net Case Managers may focus on one area of responsibility, based on community and program needs and contract/funding source requirements.

Duties/Responsibilities:

Direct Services – Provide quality, trauma-informed advocacy to support the following client or program needs:

  • Engage prospective clients in an effort to link them to Safety Net Services, including but not limited to, conducting in home visits, making contact with clients via face-to-face meetings, phone and mail, maintaining communication with the referring CPS social worker, etc.

  • Engage clients in maintaining services to increase resilience, protective factors, and self-sustainability, including but not limited to, developing and monitoring case plans with families, connecting clients to services within PREVAIL and outside referrals to community resources and supportive services, etc.

  • Regularly follow up with clients to monitor their progress toward goals.

  • Crisis intervention may include providing immediate assistance in times of crisis (e.g., domestic violence, sexual assault, homelessness, mental health emergencies, etc.).

  • Advocacy may include acting as an advocate for clients, ensuring they understand their rights and the services available to them and help clients navigate complex systems and overcome barriers to accessing services.

  • Keep detailed records of client interactions, progress, and services provided.

  • Maintain a network of communication, collaboration, and cooperation between medical treatment facilities, law enforcement agencies, Victim Witness, Child Protective Services, etc.

  • Reinforce policy and procedures with staff and clients; handle challenging client situations; and other related tasks.

  • Participate in and attend regular on-going trainings.

  • Initiate and/or maintain communication with community partners, as needed and directed by Program Coordinator or other leadership members.

  • Use conflict resolution and de-escalation skills as necessary to maintain a healthy and neutral environment.

  • Community Engagement: Represent the agency at internal and external events including, but not limited to, program partnerships, funder/contract reporting meetings, multi-disciplinary team meetings, Child and Family Team (CTF) meetings, etc.; may provide educational presentations to a variety of community-based organizations and/or partners.

Administrative

  • Execute comprehensive and detailed documentation, and perform other administrative tasks, as necessary, to support both program operations and role responsibilities.

  • Provide administrative support by handling document preparation, data entry, and office organization.

  • Manage office supplies, maintain files and calendars, and oversee the distribution of incoming and outgoing mail.

Required Skills/Abilities:

  • Must support PREVAIL's mission and be dedicated to eliminating domestic violence, sexual assault, human trafficking, and youth homelessness, in addition to supporting the fundamental values of both feminism and the social work profession, including the worth, dignity, and uniqueness of all persons as well as their rights and opportunities.

  • Successful completion of Victim Assistance Training (within six months of hire).

  • Fingerprint clearance is required.

  • Experience in working with children, adolescents and families/adults.

  • Reliability and consistency with attendance and punctuality.

  • Ability to work effectively with individuals from diverse backgrounds and cultures, and awareness of and sensitivity to the cultures represented in San Joaquin County.

  • Ability to productively work both autonomously and as a team member.

  • Complex Problem Solving identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.

  • Ability to effectively communicate and work with key stakeholders, including but not limited to government partners, community and individual supporters, executive leadership, and the Board of Directors.

Education and Experience:

  • Bachelor's degree with a major in psychology, sociology, or related field in lieu of bachelor's degree:
  • 60 completed semester units with a major course in psychology, sociology, or related field plus two years of experience providing case management services to children and families. Experience shall be verified as having been performed as a paid or volunteer staff person whose duties required direct supervision.

  • Reliable transportation, valid California Driver's License, and proof of insurance.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.

  • Must be able to lift 50 pounds at times.

  • Must be able to ascend and descend up to three flights of stairs throughout a typical workday.
