Job Overview
We are seeking a compassionate and dedicated Hospice Nurse Practitioner to join our team. In this role, you will provide high-quality, patient-centered care to individuals with life-limiting illnesses and their families. You will be responsible for assessing patients' needs, developing care plans, and delivering comprehensive palliative care in various settings including homes, assisted living facilities, and nursing homes. Your expertise in hospice and palliative medicine will be essential in ensuring comfort and dignity for our patients during their end-of-life journey.
Duties
- Conduct thorough patient assessments to determine medical needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure effective communication among caregivers.
- Administer medications, perform procedures such as IV infusions, suturing, and catheterization as needed.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life decisions.
- Manage acute pain effectively using appropriate interventions including botulinum toxin treatment and dermal fillers when necessary.
- Document all patient interactions accurately in electronic medical records (EMR) systems such as Epic or Cerner while adhering to HIPAA regulations.
- Participate in clinical research initiatives to improve patient outcomes in hospice care.
- Engage in telehealth consultations as required to enhance accessibility for patients.
- Ensure compliance with NCQA standards and best practices in hospice & palliative medicine.
Experience
Candidates should possess a strong background in nursing with specific experience in hospice & palliative medicine, geriatrics, or acute care settings. Preferred qualifications include:
- Critical care experience or experience in a Level I or II trauma center.
- Familiarity with medical documentation practices including CPT coding, ICD-9/ICD-10 coding, and utilization management.
- Experience working with diverse populations including pediatrics, geriatrics, and individuals with developmental disabilities.
- Proficiency in performing physical examinations, vital sign assessments, phlebotomy, specimen collection/processing, and airway management techniques.
- Knowledge of medical terminology related to infectious disease care, behavioral health, pain management, and home care practices is essential.
