Job Responsibilities and Duties:
  • Plan, develop, and manage social services programs within the facility.
  • Assist residents with admissions, Medicaid processes, and MDS assessments.
  • Provide emotional support and counseling to residents and families.
  • Coordinate community resources and resident appointments as needed.
  • Maintain compliance with state regulations and professional standards.
Requirements:

Licensure/Certification:

  • Education:
  • Minimum of a Bachelor's degree in social work; OR
  • Minimum of a Bachelor's degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
  • Social work degree or equivalent
