We are seeking an experienced
Intake Registered Nurse / RN - for an office based role
to join a
certified home health care agency in Brooklyn
. The Intake RN plays a key role in coordinating referrals, processing home care services, and ensuring a seamless transition of patients into care. This position requires a detail-oriented professional with strong communication skills and a solid understanding of CHHA operations and compliance.

Responsibilities

  • Receive, review, and process referrals for appropriate home care services.
  • Maintain consistent communication with referral sources, physicians, and internal departments.
  • Ensure timely and accurate documentation within the agency's Electronic Medical Record (EMR) system.
  • Support patient admissions by verifying clinical and insurance eligibility.
  • Promote the agency's mission and values through excellent customer service and professional interactions.

Qualifications

  • Current New York State RN License (required).
  • Minimum of 2 years of Certified Home Health Agency (CHHA) experience (required).
  • Proficiency with Allscripts (required).
  • Experience with HomeCare HomeBase (HCHB) or similar EMR systems (required).
  • Strong computer skills and attention to detail.
  • Excellent communication and organizational abilities.
  • Ability to manage multiple priorities in a fast-paced environment.
