Great opportunity for experienced RN Complex Care Case Manager

What you will do:
This is a dynamic role for an expert Case Manager to manage an assignment of complex cases on adult floors of The Johns Hopkins Hospital. You will work closely with Social Worker, Physician advisor, and Care Coordination leadership to ensure quality and efficient coordination of care.
In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Lead Case Manager-Complex Care provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. Manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family, and care team.

Who should apply:
- Completion of a BSN program from an accredited school of nursing required. Master of Science in Nursing (MSN) preferred.
- Minimum of five (5) years' nursing experience required. Minimum of three (3) years' case management and/or discharge planning experience.
- Current Maryland License as an RN or licensure in a compact state that allows practice in Maryland.
- Accredited Case Manager _ Registered Nurse (ACM – RN) / Certified Case Manager (CCM) required within first 6 months of hire
Salary Range: Minimum 46.57/hour - Maximum 72.18/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
