Position Summary

The Screener, Assessor, and Navigator serves a critical role within the NYS Medicaid 1115 Waiver by identifying eligible individuals, conducting comprehensive assessments, and guiding participants through healthcare and community-based services. This position integrates screening, assessment, and navigation functions to address health-related social needs, improve care access, and support positive health outcomes.

Key Responsibilities

● Conduct eligibility screenings for NYS Medicaid 1115 Waiver services using approved screening tools

● Perform comprehensive assessments addressing medical, behavioral health, and social determinants of health (SDOH)

● Identify participant needs related to housing, food insecurity, transportation, employment, and other HRSN areas

● Develop participant-centered service plans in collaboration with care teams and community partners

● Navigate participants through healthcare systems, social services, and community-based resources

● Provide education and support to participants to promote engagement and self-advocacy

● Manage an assigned caseload and maintain consistent participant contact

● Document screenings, assessments, referrals, and outcomes accurately in electronic case management systems

● Coordinate referrals and follow-up to ensure service linkage and continuity of care

● Communicate effectively with providers, service provider specialists, community organizations, and internal staff

● Participate in case conferences, interdisciplinary team meetings, and ongoing program training

● Ensure compliance with NYS Medicaid 1115 Waiver requirements, HIPAA, and organizational policies

Required Qualifications

● Associate or bachelor's degree in social work, human services, public health, healthcare, or related field preferred

● Minimum of one (1) year experience in screening, assessment, care coordination, navigation, or case management

● Strong interpersonal, verbal, and written communication skills

● Proficiency in computer systems, electronic health records, and case management platforms

● Demonstrated ability to manage a caseload and complete timely, accurate documentation

● Reliable transportation and a valid New York State driver's license

● Ability to work independently, in the field, and as part of a multidisciplinary team

Preferred Qualifications

● Experience with NYS Medicaid, Medicaid Waivers, or managed care programs

● Knowledge of social determinants of health and community-based resources

● Bilingual or multilingual abilities strongly preferred

We are seeking a compassionate and proactive Screener/Assessor/Navigator to join our team and help connect individuals to essential Medicaid services through the NYS Medicaid 1115 Waiver Program. This role involves guiding clients through the enrollment process, conducting assessments, and providing behavioral health and social support to promote overall well-being.

Responsibilities

  • Conduct initial screenings and assessments to determine eligibility for Medicaid services under the NYS Medicaid 1115 Program
  • Serve as a navigator, guiding clients through complex enrollment procedures with clarity and patience
  • Utilize motivational interviewing techniques to engage clients, identify their needs, and foster trust
  • Provide crisis management support when necessary, ensuring clients receive appropriate immediate assistance
  • Collaborate with community partners and service providers to coordinate care plans and workforce development efforts
  • Document all interactions thoroughly, maintaining accurate records of assessments, referrals, and client progress
  • Stay informed about Section 8 housing programs, addiction counseling options, behavioral health resources, and social work best practices

Experience

  • Prior experience in social work, behavioral health, or related fields such as addiction counseling or crisis management is highly preferred
  • Knowledge of NYS Medicaid policies, Section 8 housing programs, and workforce development initiatives is a plus
  • Demonstrated ability to conduct motivational interviewing and manage crisis situations effectively
  • Strong communication skills with an energetic approach to engaging diverse populations
  • Familiarity with social service systems and community resources relevant to Medicaid recipients
