Job Description:

As a Hospice LVN, you will provide high-quality, patient-centered care to individuals in their homes or facilities. Your role will involve working closely with a multidisciplinary team to ensure the comfort and dignity of our patients during their end-of-life journey.

Key Responsibilities:

  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient conditions and report changes to the healthcare team.
  • Provide emotional support and education to patients and their families.
  • Assist with daily living activities and personal care needs.
  • Collaborate with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans.
  • Maintain accurate and timely patient records.

Qualifications:

  • Current LVN license in Tx.
  • Minimum of 1 years of nursing experience, preferably in hospice or palliative care.
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and patient-focused approach to care.
