Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and injections with proper aseptic technique.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing to ensure patient safety and comfort.
- Monitor patients' conditions closely, utilizing knowledge of physiology and anatomy to identify changes requiring urgent attention or escalation.
- Provide specialized care for patients with dementia, Alzheimer's disease, or memory care needs in settings like assisted living or memory care units.
- Support acute pain management strategies and participate in case management activities to optimize patient outcomes.
- Collaborate with multidisciplinary teams in emergency rooms, ICU units, trauma centers (Level I & II), pediatric units like PICU or Neonatal ventilator care, and other medical specialties including internal medicine, geriatrics, behavioral health, and infectious disease care.
- Document all patient interactions thoroughly in compliance with HIPAA regulations and medical documentation standards.
- Engage in discharge planning and patient education to promote recovery at home or within community-based programs.
- Maintain infection control practices and ensure sterile techniques during procedures such as phlebotomy, venipuncture, specimen collection/processing, and catheter care.
- Participate in training on ICD-9/10 coding for accurate medical billing and documentation purposes.
- Demonstrate proficiency with EMR/EHR systems like Athenahealth or eClinicalWorks to streamline patient records management.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience in diverse clinical environments such as hospitals (including ICU or emergency room), nursing homes, hospice care, or outpatient clinics.
- Critical care experience preferred, especially within Level I & II trauma centers or acute care settings.
- Familiarity with medical terminology, physiology knowledge, and diagnostic evaluation techniques essential for comprehensive patient assessment.
- Hands-on experience administering injections, IV insertion/infusion therapy, tube feeding management, and catheterization procedures.
- Proficiency with EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-9/10) is advantageous.
- Strong understanding of HIPAA regulations to ensure patient confidentiality at all times.
- Ability to work effectively with diverse populations including seniors requiring geriatrics or memory care services; children needing pediatric or neonatal ventilator support; individuals with developmental disabilities or behavioral health needs.
- Excellent communication skills for triage assessments and interdisciplinary collaboration in fast-paced environments such as urgent care or hospital medicine units.
- Basic life support (BLS) certification required; additional certifications such as ACLS are a plus.
- Physical ability to stand for extended periods, assist patients with mobility aids like Hoyer lifts when necessary, and perform physically demanding tasks safely.
