Job Description

The Clinic Coordinator provides administrative coordination and support to the School-based Health Center (SBHC) and/or medical outreach team. The Coordinator is responsible for managing patient registration, scheduling, and check-in as well as operating as the main site contact for school partner referrals and coordination when in support of an SBHC location. The Coordinator works with the SBHC Program Manager, Business Development Director, and Clinical Teams to conduct outreach and engagement efforts to increase student, family, school, and community awareness of the outreach location to maximize utilization. When support at an outreach location is not needed, the Coordinator may work at a CHC medical site or on outreach and operations projects to support the SBHC and overarching outreach efforts for CHC, based on business needs and as directed by operational leadership.

  • Knowledge of medical terminology.
  • Ability to develop meaningful and appropriate relationships with students, school staff, and families.
  • Ability to develop successful interpersonal relationships with diverse populations including leaders, clinicians, school staff, and Community Based Organizations.
  • Ability to understand and maintain productivity expectations and generate solutions to overcome any barriers.
  • Ability to demonstrate excellent verbal communication skills and customer service.

Preferred:

  • Bilingual skills.

Education

  • High school graduate or equivalent

Preferred:

  • Associate or bachelor's Degree in a healthcare related field, or equivalent experience working 2 years of more in a healthcare setting.

Experience

  • Electronic Health Record (EHR) and/or Electronic Practice Management System experience (1 year).
  • Experience working with low-income, diverse populations (1 year).

A. Job Specific Functions/Performance

1. Greets patients, families, collaborative partners, and visitors in person and by phone in a prompt, courteous and helpful manner. Responds to inquiries, takes messages, and provides accurate information regarding schedules and services.

2. Checks in, registers, and schedules patients in the practice management system. Maintains appointment schedule and follows scheduling policies.

  1. Assists in managing patient flow in a timely and efficient manner. Interfaces with clinical teams regarding walk-in patients, cancellations, and delays to ensure smooth patient flow.
  2. Supports monitoring and reporting of vaccine equipment temperature and status, participating in vaccine audits, and ensuring up-to-date record keeping.

  3. Assists in outreach activities focused on promoting, integrating, and informing patients, students, families, staff, and the general community about the CHC services offered.

  4. Supports Cross-school and cross-location referral coordination and serve as primary point of contact for referral and service inquiries from patients, students, families, and other healthcare and basic support service agencies in the community, as needed.

  5. Under direction of supervisor, coordinates and/or performs basic educational outreach activities focused on health promotion and health education to patients, students and families.

  6. Serves as a knowledge resource for students, families, and partners and maintains collaborative partnerships with site referring agencies.

  7. Coordinates data collection for reports, audits, and quality reviews as assigned.

  8. Routes or files documents and mail appropriately.

  9. Assists with insurance eligibility verification.
  10. Orders interpreters and interfaces with outside organizations regarding patient services.
  11. Opens and closes clinic for all business hours.
  12. Maintains adequate inventory of office supplies, medical and dental supplies, and clinical forms.
  13. Performs maintenance and troubleshoot operations of office equipment and machines to include calculators, copiers, fax, printers, etc.

  14. Assists with other departments and locations when called upon.

  15. Coordinates with CHC medical and dental department teams on additional staffing needs to maintain operations at the SBHC and outreach site(s).
  16. Adheres to attendance standards in order to perform the job functions for daily operations and /or continuity of patient care.
